Jon Fields MD, MPH, MBA, CIME

Dr. Fields is a graduate of the University of Iowa Carver College of Medicine and is Board Certified in Occupational and Environmental Medicine. He completed his internship in internal medicine at the University of Iowa Hospitals and Clinics and went on to graduate from the University of Iowa Occupational Residency Program. Prior to his medical career, he has over 10 years of experience in engineering roles. In addition to his unique skillset, Dr. Fields has a business background with years in the corporate care setting, facilitating his ability to meet the specific needs of employers and employees. He has spent nine years as a staff physician in Occupational Medicine, servicing patients throughout the Cedar Valley, northern Iowa and Southeast Minnesota. He is also a certified Medical Review Officer (MRO), Federal Motor Carrier Safety Administration (FMCSA) Medical Examiner and Senior Federal Aviation Medical Examiner. His specific clinical interests include sports medicine and worksite injury prevention.
Christine Clay – Operations/Clinic Supervisor

 Christine is the clinic supervisor at Waverly Industrial Rehab as well as the Head  of Operations at Summit Industrial Medicine.

​Christine has been working in the field of Occupational Medicine for the last 15 years in various capacities.  She utilizes her vast experience in the field to drive our clinic operations.  You will find her at the front desk, performing drug screens, scheduling, billing, and basically a little bit of everything!

In her spare time she enjoys spending time with family and golfing. 
Michelle Sager – Clinic Administrator

Michelle is the face of the front desk in our clinic and handles all aspects of scheduling with both WIR and Summit industrial Medicine. Outside of work she enjoys reading, crafts, puzzles, being outdoors and going to amusement park with her family.  She has a husband of 34 years, four children and five grandchildren.  ​
Katie Franzen – ATC

Katie is a 2010 graduate of Iowa State University. She has both a master’s degree in kinesiology and a doctorate in adult and higher education. She is a certified athletic trainer who is proficient in many aspects of our clinic including vision testing, audio, EKG, PFTs, drug screens, breath alcohol testing (BAT) in addition to her patient care.  She is instrumental in our FAA, DOT and physical examinations. Outside of work she enjoys spending time with her husband, 1 year old son and two dogs. 
Maria Farmer – ARNP

Maria completed her nurse practitioner and Acute care training at St. Louis University. She has 13 years of emergency medicine experience as a nurse practitioner. She lives in Waverly, Iowa and enjoys family time and water sports.
