Independent Medical Examinations require a $975 prepay at the time of scheduling.
Medicolegal Case Review, Depositions and Complex Take Over Care/2nd Opinion Cases require a $725 prepay for the first hour of work. Please contact us to be sent an invoice and arrange for records to be sent to Dr. Fields.
Additional work is billed at $175 per 15 minute unit.
Flat Fees for impairment rating only visits $795 (treating) and $895 (non-treating).
Contact us for pricing for post-offer physicals, fit for duty tests, pulmonary function testing or other needs. Drug testing and Breath Alcohol Testing is available in Waverly.
Pricing for FAA Physicals is as follows:
1st Class $297
2nd & 3rd Class $235
EKG (if needed) $85
Basic Med $195
Waggoner stand-alone color vision testing $55
No show fees MAY be charged for failure to provide notice and not attending scheduled appointments, depending on the circumstances. Independent Medical Exam no show fees are $575. Other specialty exam no show fees are $329. Clinic visits could be charged a no show fee of $197. Failure to pay a no show fee will result in inability to reschedule that visit.
We ask that individuals arrive 30 minutes prior to their appointment for IME’s and specialty examinations to fill out paperwork.
We reserve the right to reschedule visits for patients that arrive over 15 minutes late.
Medical Record Fees are equal to the actual cost of production. Fee not to exceed the rates established by the workers compensation commissioner for copies of records in workers compensation cases:
$20.00 for the first 20 pages
$1.00 per page for pages 21-30
$0.50 per page for pages 31-100
$0.25 per page for pages 101-200
$0.10 per page for pages over 200

A patient or a patient’s legal representative or a patient’s attorney is entitled to one copy free of charge of the patient’s complete billing statement, subject only to a charge for the actual costs of postage or delivery charges incurred in providing the statement. If requested, the provider shall include an affidavit certifying that the records, images, or billing statement produced are true and accurate copies of the originals for an additional fee not to exceed ten dollars. Fees charged are not subject to a sales or use tax.
